Patient Registration Requirements
Welcome to the Metropolitan Family Health Network, Inc. Below is a list of items that will assist the Metropolitan Family Health Network staff to expedite your registration process in a timely manner:

1. Name, address, date of birth, contact phone number
2. Picture identification (for example, driver’s license, state-issued non-driver’s license, passport, Green Card are the most helpful, but if you don’t have one of these bring what you have)
3. Original Social Security card
4. Proof of income (for example, current pay stub, bank statement, W2, etc.)
5. Proof of residential address (for example, a telephone, utility, and/or cable bill)
6. Letter of Verification of Support from the individual who supports you financially
7. Insurance claim forms and insurance identification cards

Requisitos Para la Registracion del Paciente
Bienvenidos al Centro de Salud Familiar Metropolitan e Incorporados (Metropolitan Family Health Network, Inc.) A continuacion encontrara una lista de documentos que debe de traer para facilitar el proceso de registracion:

1. Nombre, direccion, fecha de nacimiento, y numero de telefono de contacto en caso de emergencia
2. Identificacion personal con fotografia (licencia de conducir, pasaporte, targeta de residencia, etc.)
3. Tarjeta de Seguro Social
4. Prueba de ingresos (mas reciente talonario de pago, reporte del estatus actual de su cuenta de banco, W2)
5. Si no trabaja, provea Carta de Verificacion de ingresos de quien lo alluda economicamente.
6. Tarjetas de seguros de salud y formularios especiales si el seguro de salud require pagos.
